2nd Meet up for Donation Booklet Collection

The donation booklet collection this week will be at Armenian Street!!

Come meet us and collect your donation booklet as follow:
Date: 10th May 2014 (Saturday)
Time: 5pm - 7pm
Venue: Armenian Street Booth #22

Please kindly print out the Camper Registration Form 2014 (http://bit.ly/RegistrationForm2014), complete and submit when you collect your donation booklet so we can officially register you in for SEEDs 30-Hour Famine DIY-Camp 2014. Pls bring along RM20 for your camp registration fees on that day.

If you are 18 years old and below, additionally please help to ask your parents to filled another Parental Consent Form 2014 (http://bit.ly/ParentalConsentForm2014) submit on that day as well.

Campers Checklist upon donation booklet collection day.
1) Camper Registration Form 2014
2) Parental Consent Form 2014 if you are under 18 years old
3) Camp Fees RM20
4) Representatives are allowed to help collect for 5 friends including himself/herself
(together with item 1, 2 and 3 of friends whom they are collecting on behalf)

如果您未满18岁，请附上家长同意书(http://bit.ly/ParentalConsentForm2014) 一并呈交。

您需要于这个星期六呈交：
1）2014营员报名表格
2）家长同意书若您未满18岁
3）营费 马币20令吉
4）若您不能亲自前来，请派代表来呈交，一个代表可以帮最多4 个人呈交以上表格及营费。
